Directory Registration

Joining the healthcare provider (HCP) Directory is one of the benefits available to approved healthcare providers once they join the CFMS. Qualifying practitioners must create a directory profile, fill out the application, and submit it for approval. Once approved, practitioners will be notified, and their profiles will updated on the HCP Directory.

Directory Profile: Directions

To create your directory profile, use the link below. First-time users will create a profile with their name and email address. The application form will automatically load, and you can fill out the required fields to get the ball rolling on Directory approval.

Directory profiles will be verified within 48 hours of submission. Someone from the CFMS team may contact you for clarification or additional information.

  • The CFMS application is extensive and requires you to upload licensing credentials. You will need to upload an image file of your state license where your practice is located. Additionally, you’ll need to upload an image file of your CrossFit credential.
